Health Care Referral Policy for Employees of Service Contractors

PURPOSE:

To provide information and access to quality health care resources in our community for on-site employees of service contractors of the University of Miami and their dependents. 

 

DEFINITIONS:

On-site Employee of Service Contractor of the University of Miami (Contractor Employee)-An employee who works a minimum of 30 hours a week and has been employed for three consecutive months by a contractor who provides services onsite to the University of Miami.

Dependent-A spouse recognized under Florida law or dependent child as defined.  A marriage license is proof of dependency for a spouse.

Dependent Child-A natural, adopted or stepchild receiving 50 percent or more support from the Contractor Employee.

 

HEALTH CARE REFERRAL OFFICE:

The office designated to assist Contractor Employees arrange for preventive services, screenings and physician appointments at various health care providers throughout Miami-Dade County.

 

PROCEDURE:

Appointments with Office of Health Care Referral Coordinator:

On-site employees of service contractors should contact the Health Care Referral Office at 305-284-8195 to make an appointment or request a referral. For Contractor Employees not covered by health insurance, the University of Miami will reimburse the contractor employee or dependent family member the cost of the initial visit to a community clinic or personal physician.  The maximum reimbursement is $75.00 and the receipt must be presented for reimbursement.

 

LAYOFF:

On-site employees of contractors that are placed on layoff during winter and summer breaks are eligible to continue to utilize the service of the Health Care Referral Office.

 

TERMINATION:

Upon the effective date of termination of a Contractor Employee, health care referral assistance will cease.
