Change of School/College

Students who would like to change schools/colleges must request this through a Change of School/College Form obtained from their current academic school/college.

1. This form must be filled out by your current academic school/college and contain the appropriate, authorized signatures approving the change. 
   
   
2. This form will then be sent to the new school/college for the appropriate, authorized signatures approving the change.
   
   
3. The completed form will then be forwarded to the Office of the Registrar for processing.

Change of Major

Students who would like to change majors must request this through a Change of Major Form obtained from their current school/college.

1. This form must be filled out by the current academic school/college in which you are enrolled and contain the appropriate, authorized signatures approving the change. 
   
   
2. The completed form will then be forwarded to the Office of the Registrar for processing.
