Insurance Information

 

Domestic Students


Domestic students enrolled in six or more credit hours per semester (or considered full time, including graduate students enrolled in a 700/800 level class) are required to obtain adequate health insurance (see exceptions). The annual premium for the health insurance plan offered through the Student Health Service is added to each student's fees. Domestic students with adequate alternative coverage may request cancellation of the insurance fee by submitting a Domestic Insurance Cancellation Form, or via MyUM. Students with limited out of area coverage or otherwise inadequate coverage are urged to carefully review their options before waiving the Student Health Service sponsored insurance plan. Deadlines to waive the insurance are July 25th for the Fall semester, January 25th for the Spring semester, April 25th for Summer I and June 25th for Summer II.

Insurance cancellation requests must be renewed each academic year via MyUM .

Insurance Rates for the upcoming academic year are generally available by June 1st. Part-time students and others not routinely eligible for care at the Health Center who chose to be insured will be charged the University fee in addition to the annual premium for health insurance coverage.

Optional coverage for dependents can be requested at the time the student is first able to enroll in the plan (within 14 days of the start of the semester) or within 30 days of termination of other similar coverage, or because of any of the following events; birth, legal adoption, placement for adoption, marriage, legal guardianship, or court or administrative order. Renewal of dependent coverage is the responsibility of the student / dependent and must be requested prior to the termination of the current policy in order to prevent a lapse in coverage. Late requests for renewal must be received within 14 days of the start of the semester. Charges are paid directly to the Student Health Service, payment by credit card is preferred. Students who previously waived the insurance charge can reinstate the insurance prior to the Spring semester by completing the reinstatement request form and checklist. Coverage can also be reinitiated at the start of the Fall semester by choosing not to re-waive the charge, or at other times during the academic year, if within 30 days of termination of other similar coverage, by completing the reinstatement request form and checklist. Documentation of termination may be requested.

The deadline for reinstatement of insurance is January 25th for the Spring semester. Please do not consider your reinstatement complete until reinstatement has been verified via MyUM, the charge has been posted to your student account and all charges on your account have been paid.

Students scheduled to graduate at the end of the Fall semester can request to be charged for the Fall semester only by completing an insurance exception form by September 1st. Students who request to be charged for the Fall only, and remain enrolled, will be subject to the Spring/Summer insurance fee.



 

International Students


All international students are required to enroll in the University sponsored health insurance program. The annual premium for this coverage is added to each student's fees. Optional coverage for dependents can be requested at the time the student is first able to enroll in the plan (within 14 days of the start of the semester) or within 30 days of termination of other similar coverage, or because of any of the following events; birth, legal adoption, placement for adoption, marriage, legal guardianship, or court or administrative order. International Students / Optional Practical Training


International students who are no longer enrolled at the University but remain in the United States completing Optional Practical Training may still be insured under the Student Insurance policy if they originally purchased a plan that continues beyond the completion of their studies. To verify the termination date of your insurance plan please call (800) 436-7709 or log on to www.myuhc.com. Although insurance coverage may continue during OPT training, access to the Student Health Service is limited to enrolled students. Expenses incurred off-campus will be subject to deductibles, co-payments and co-insurance. TA/GA/RA Insurance


TA/GA/RA's are required to obtain adequate health insurance according to the guidelines described above for other Domestic and International students. TA/GA/RA's with an active assignment can pay for their insurance via payroll deduction by requesting this option under the employment tab at MyUM. If the payroll deduction option does not appear on your MyUM screen, please contact your department to verify the status of your assignment. The payroll deduction option will not appear on your MyUM screen until your student assignment  has been processed by your department and entered by the Office of Student Employment. Although insurance waiver requests must be submitted by July 25th, the deadline to select the payroll deduction via MyUM is October 1st. A health insurance credit is available for Coral Gables TA/GA/RA's, Fellows and Trainees enrolled in PhD and DMA programs. Graduate Medical Students


Graduate Medical Students who are admitted to the graduate programs in Biochemistry and Molecular Biology, Cancer Biology, Microbiology and Immunology, Molcular Cell and Developmental Biology, Molecular and Cellular Pharmacology, Neuroscience and Physiology and Biophysics or Interdisciplinary Biomedical Studies (IBS) and who are in good academic standing are supported financially throughout the Department of Graduate Studies. Support includes 80% of the University of Miami student health insurance premium per student. Insurance Plan 2008 - 2009


The Health Insurance Plan for 2008-2009 is offered by United Healthcare. This plan is designed to be used in conjunction with the services of the Student Health Service and Counseling Center. To obtain greatest benefits, you will need to utilize the Student Health Service first, where treatment will be administered or referral issued; except in the case of medical emergency, maternity, when away from campus or when the Student Health Service is closed.

There are no co-payments for clinical and diagnostic services provided at the Student Health Service or Counseling Center. Co-payments and coinsurance may apply to services received elsewhere. Higher out of pocket costs apply when using out-of- network versus in-network providers. Prescription drug benefits are also greatest at the Student Health Service Pharmacy (lower co-payments and hormonal contraception benefits). See the insurance brochure for more information. All students including those with limited out of state coverage, and otherwise inadequate coverage are encouraged to review their options carefully before choosing to waive the insurance charge.

Changes in this year's health plan include:

  • Low cost generic medications are now available for a $4.00 co-payment for a 30 day supply or $12.00 for a 90 day supply at the Student Health Pharmacy.

  • Tier I prescription drug co-payment has been reduced to $10.00 per 30 day supply.

  • Lower co-insurance for inpatient services and outpatient surgery at University of Miami Hospital, Bascom Palmer Eye Institute and Sylvester Comprehensive Cancer Center (10% co-insurance at these UM facilities versus 20% co-insurance at other in-network facilities)

The following changes instituted for 2007-2008 continue:

  • Hormonal Contraception is covered if dispensed at the Student Health Service Pharmacy.
  • Lowest pharmacy co-payments and greatest prescription coverage are  available at the Student Health Service Pharmacy.
  • Smoking cessation medications are covered at the Student Health Service Pharmacy.
  • Referrals are required in order to obtain the best benefits and coverage (out-of-network coverage does not require referral)
  • The HPV vaccine is covered with a 50% co-payment, if administered at the Student Health Service.
  • Routine eye exams are covered for graduate students only (one per year, subject to co-payment, eyewear is not covered). Graduate students who are financially registered may call Dr. Lloyd Schneider, D.O. at (305) 662-9300 to schedule an appointment after the start of the semester.
 

Travel Benefits


Through participation in the UnitedHealthcare's Student Plan, you are eligible for global emergency assistance services when traveling abroad. Services are provided by Worldwide Assistance Services, Inc. (800)898-3344.

Coverage limits under the Premium Plan include:

  • Medical Evacuation: Covered up to $100,000.

  • Medical Repatriation: Covered up to $25,000.

  • Non-Medical Repatriation: Covered up to $5,000

  • Return of Remains: Covered up to $10,000

  • Transportation to visit a hospitalized student: Covered up to $5,000

  • Transportation to return companion: Covered up to $5,000

  • Transportation to return minor children: Covered up to $5,000

  • Hospital admission guarantee: $5,000

  • 24 hr AD&D: Covered up to $10,000

Please keep in mind, should you need medical care while abroad, payment may be required at the time of service. Please keep all invoices and receipts to allow you to submit for reimbursement of any eligible expenses upon your return. You may also wish to purchase additional travel insurance for services not provided by this plan.

 

Getting the Most from Your Insurance Plan


Read the Health Insurance Brochure and the Health Center Fact Sheet

Understand basic coverage, co-payments and deductibles, policy limits, exclusions, and pre-existing condition limits. Please understand that certain medical costs may not be covered by the policy, and that verification of insurance does not constitute a promise to pay non-covered charges. Contact the insurance company, or the Health Center with any questions.

Visit the Health Center for primary care services.

Services provided at the Health Center will usually be more affordable than those provided elsewhere. If you are referred to a specialist, consider returning to the Health Center to have necessary lab tests and basic X-rays. These services and other follow up care are likely to be more affordable at the Health Center.

Use the Health Center pharmacy whenever possible, and encourage your physician or other provider to prescribe generic, or other affordable medications. Call the Health Center as soon as possible after any emergency room visit or hospitalization.

Follow-up care can often be obtained from Health Center staff.

Save copies of all bills, checks, credit card receipts, E.O.B's (explanation of benefits forms) and any other written documentation.

Determine your share of all charges, and pay promptly to avoid penalties or referral to collection agencies.

Encourage your physician, hospital or other provider to promptly submit bills, and to respond to requests for clarification or additional information.

Insurance Cards


Insurance cards are distributed early each semester, but will not be available until you have financially registered and paid your insurance charge, and all other fees. Although insurance information is transmitted frequently to United Healthcare, there may be a delay of 2-3 working days between financial registration and processing of your information in the United Healthcare system. Please update your local address at MyUM to assure that the card is mailed to your preferred location. Temporary cards may also be available at www.myuhc.com after you have completed financial registration. You will need your United Healthcare subscriber number which can be obtained by calling (800) 436-7709. Please refer to group # 709872 when calling to obtain your subscriber number. If you have not received your card by September 30 for the Fall Semester, January 30 for the Spring Semester, or June 30 for the Summer Session, please contact United Healthcare.
