Payroll Forms

  1042s Request Form  To be used for requesting a duplicate 1042s Form
     
  Check Replacement Request Form To be used for replacing lost payroll checks
     
  Employee Override Authorization Form Used by departments to authorize regular and/or overtime charges to an override account
     
  Foreign National Information Form To be completed by foreign students and employees on visa types "F", "J" or "H" to determine residency and tax status
     
  Manual Check Form Payroll check request for missing paycheck
     
  Online Payroll Direct Deposit via myUM– Used by all UM employees to request payroll deposit to new accounts, changes to existing deposit amounts, bank accounts and routing numbers.  Log on to myUM at   http://myum.miami.edu  and follow these instructions:
  1) Click on the  Employee  tab
  2) Select  Payroll, HR & Misc.
  3) Click on Direct Deposit listed under the Employee Information section.
     
  Payroll Direct Deposit Form  For use by newly hired employees who have not yet established a CaneID
     
  PaystubRequest Form  to be used for requesting paystubs that are not available on myUM.
     
  Revised Timesheet
     
  Revised Timesheet-Hospital Employees
     
  Revised Timesheet-Student Employees
     
  W2 Request Form To be used for requesting a duplicate W2 Form
     
  W4 Form  Employee's Withholding Allowance Certificate-used by employees to determine the amount of federal income tax to be withheld from their paycheck
   
  W5 Form To be used if an employee is eligible to get part of the Earned Income Credit in their
   
  W7 Form Used by a foreign student or visitor to apply for an individual taxpayer identification number
   
  W8 BEN Form  Used by non-resident aliens to claim a tax treaty exemption and/or to claim foreign status (Instructions: www.irs.gov/pub/irs-pdf/iw8ben.pdf)
