Forms DS-2019 for spouse and/or minor children of J-1 scholars


• If you are a current J-1 scholar under the auspices of the University of Miami’s J-1 program, your legally married spouse and children under the age of 21 may qualify for J-2 dependent status. If you wish to request Form(s) DS-2019 for your spouse and/or children under 21, please provide the following information to Claudia Zitzmann, Associate Director, International Student and Scholar Services (ISSS), via e-mail (czitzmann@miami.edu) for each dependent spouse and child:

    • Last name
    • First name
    • Middle name(s)
    • Date of birth
    • City of birth
    • Country of birth
    • Country of citizenship
    • Country of legal permanent residency

• If your Form DS-2019 already shows sufficient funding to support the dependent(s) for which you are requesting a Form DS-2019, then you will not have to show additional proof of funding before Form(s) DS-2019 can be issued.

• However, if your Form DS-2019 does not show sufficient funding to support the dependent(s) for which you are requesting a Form DS-2019, then you will have to show additional proof of funding before Form(s) DS-2019 can be issued.

           Minimum required funding for a scholar program of 12 months duration is as follows:

           Single scholar              $17,500 + cost of health insurance ($1,604/year)*

           Spouse                          $11,000 + cost of spouse’s health insurance
                                                   (Spouse under age 25: $3,456/year)*
                                                   (Spouse age 25 – 34: $4,204/year)*
                                                   (Spouse age 35-44: $5,352/year)*
                                                   (Spouse age 45 and older: $6,848/year)*

           Each child                     $2,700 + cost of child’s health insurance ($1,444/year)*

           *Please note that these insurance fees are subject to change.  The insurance fees quoted here refer to Insurance for Students, Inc. rates from August 15, 2007 to August 14, 2008.

• Once your ISSS advisor has received the required information regarding your dependent spouse and/or children (and additional proof of funding, if necessary), she will add your dependent(s) to your SEVIS record and issue Form(s) for your dependent(s). Upon issuance of Form(s) DS-2019, you should collect the document(s) for your dependent(s) from ISSS and send the document(s) to your dependent(s) in your home country so that they can apply for J-2 visa(s) through the U.S. Embassy.

• When your J-2 dependents arrive in the U.S., please bring their passports, Forms DS-2019 and I-94 to ISSS for copying of their documents or send or e-mail copies of the following documents for each dependent:

    • Passport biographical information page
    • Passport expiration date page
    • Passport page bearing U.S. entry stamp
    • J-2 visa page
    • Form DS-2019
    • Front and back of Form I-94

• Please be mindful that as a J-1 scholar, you are responsible for making sure that your J-2 dependents have the required health insurance coverage, including medical evacuation and repatriation coverage, for the entire duration of their stay in the U.S. For specifics on the J-1 and J-2 health insurance requirement mandated by the U.S. Department of State, please look at the information on this website under Health insurance requirement for J-1 scholars and their J-2 dependents.

• Please allow three business days for processing of your request for Form(s) DS-2019 for your spouse and/or minor children.
