The University of Miami School of Medicine values merit, need, and diversity in making scholarship decisions. To be considered for a scholarship you must complete and submit this form to the Scholarship Committee. Your responses will be used only in making scholarship awards and will NOT become a part of your permanent record. To make sure your entries do not exceed the stated size limits, you should compose your responses in a Word document and then copy and paste the text to the appropriate box below. All information is subject to verification.

Our scholarship funds are extremely limited and submitting this form does not guarantee that an award will be made to you. Furthermore, the review process can be quite lengthy. If an award is made we will contact you immediately. If you do not hear from us, you can assume that you have not received an award.
